REQUIRED DOCUMENTS FOR NEW AND TRANSFER STUDENTS FOR THE 2008/09 SCHOOL YEAR GRADES K – 8
Dear Parent(s)/Guardian(s):
Listed below you will find all of the documents that are needed before your child(ren) will be considered for enrollment at Santa Maria del Popolo School for the 2008/2009 school year.
Required documents that must be filled out completely when registering for ALL NEW AND TRANSFER STUDENTS.
· Tuition Management System/Contract · Lunch Monitor Program Reply · Bussing form (if applicable K-8) · Certificate of Live Birth (Must be 5 by September 1st) · Baptism Certificate (if applicable) · New student registration form (applies to any student new to our school) · Current Medical and Dental forms (out of state families must have an Illinois form) (these forms must be on file with the school prior to the 1st day of school)
ADDITIONAL DOCUMENTS REQUIRED FOR TRANSFER STUDENTS (GRADES 1 – 8) ONLY THAT MUST BE TURNED IN AT THE TIME OF REGISTRATION:
· Transcript of grades from current school · Copies of all test scores
REGISTRATION FEES FOR NEW FAMILIES:
· ONE STUDENT FAMILY - $125.00 PER FAMILY
· TWO STUDENT FAMILIES - $175.00 PER FAMILY
· THREE OR MORE STUDENT FAMILIES - $225.00 PER FAMILY
Non-Refundable Registration fee is due at the time of registration. Checks should be made payable to Santa Maria del Popolo. |
